Build and Customize Your Reporting Form

Build and customize your reporting form by adding, editing, and organizing questions shown to the whistleblower

In the Form Builder, you define what information the whistleblower provides when submitting a report. You can edit the default fields, remove what you don’t need, and add your own questions.


Steps

  1. Go to Reporting channels

  2. Select your reporting channel

  3. Open the Form Builder tab

  4. Add, edit, or reorder form elements


Default fields in your form

When you open the Form Builder, you will already see a set of default fields.

Subject

Click on Subject to rename the field and update it for each available language.

Choose how you would like to report

This section lets the whistleblower choose whether to report:

  • Confidentially

  • Anonymously

When Confidentially is selected, the Name and Phone number fields will appear in the form. When Anonymously is selected, these fields will be hidden.

Description

This is where the whistleblower describes the case. You can rename this field by clicking on Description.

Departments and categories


These fields control where the report is assigned.

  • Select departments associated with the case

  • Select categories associated with the case

Name and Phone number


Both fields can be removed by clicking the X icon. Phone number is an optional field. Both fields will appear if the whistleblower chose to report confidentially.

Recipients


This determines which case handler the report will be sent to. The available recipients depend on the selected category.

Files


Allows the whistleblower to upload files as part of the report.


Edit and organize fields

You can control when each field is shown, how it appears, and what it contains.

Reorder fields

Click and hold the drag icon to move fields and change the order of your form.

Field settings

Click the settings icon to control when a field is shown. You can choose whether the field should appear when the reporting method is:

  • Written

  • Oral

  • Both

This allows you to show specific questions depending on how the whistleblower submits the report.

Translate fields

Click the translation icon to update the wording in different languages.

  • For most fields, this allows you to translate the field text

  • For sections like “Choose how you would like to report”, you can translate multiple parts such as:

    • Title

    • Confidential (header, preview, expanded text)

    • Anonymous (header, preview, expanded text)

Remove fields

Click the X icon to remove a field from the form.


Add custom fields

You can add additional questions to collect more information.

  1. Click Add element

  2. Choose:

    • Predefined question (ready-made questions)

    • Custom element (your own fields)

  3. Select the field type

  4. Click Create
