How to create a form

This guide will give you all the tips and ticks you need to create your first form within Forms for Confluence.

SETTING UP YOUR FORM ⚙️

Click on "Apps" in your top menu bar and select "Forms for Confluence" from the dropdown. Select "Create a new form".

Fill in the details for your new form. These include:

  • Your form's name (consider setting a naming convention for form names)

  • A description of your form

  • The space your form belongs to

  • Your form's parent page

  • The title of the page where your form will live

  • Any collaborators you want to add

Then click “Continue”.

Once you've entered the details to create your form, you'll be redirected to the page where your new form lives.

  • You should see an empty macro placeholder. Click on it and select the pencil icon ("edit”) to open up the intuitive form builder.

  • Click "+ Add question" and choose a form field from the menu. You can pick from many question types to create your form.

  • In the form preview on the left, click "Add a question" and type your question. You can add a description (optional) by clicking "Add a description".

  • On the right side, you can add the answers to your question and check the box to make an answer mandatory. For some question types (like Dropdown and Checkbox), you can allow users to select more than one answer by checking "Multi-select".

Once you've finished adding questions, hit "Save”.

When you're ready, click the blue "Update” button, and you're ready to share your new form. 🙌
