Adaptive Notes carries your corrections into the notes that follow, automatically. Answers to the most common questions below.
â
What is Adaptive Notes?
Adaptive Notes is a Tandem feature that carries your corrections into the notes that follow. When you edit a note, Tandem picks up on the pattern and applies it to future notes, so each new note starts closer to how you write. There's nothing to set up. It starts from your first corrected note, and the effect gets stronger the more notes you correct.
Adaptive Notes changes the form of your notes: structure, headings, wording and how detailed each section is. It doesn't carry facts about one patient into another patient's note.
â
When will I notice a difference?
Right away if you've used Tandem before. Your recent corrections are used from your first note after Adaptive Notes is switched on. Most clinicians notice a clear difference after about three corrected notes with the same template, and the effect grows the more notes you correct.
In the first week you may edit a little more than usual while your notes adapt. This settles as more of your corrections are picked up.
â
How far back does it look?
Adaptive Notes only uses edits made within your clinic's retention period. If your clinic keeps notes for a short time, or you take a long break, older corrections drop out. In that case it may take a few corrected notes before your notes feel adapted again.
â
Will my edits change my colleagues' notes?
No. Adaptation is per clinician and per template. Your edits only shape your own future notes with that same template. Each template adapts on its own, so a template you rarely use adapts less.
â
How is Adaptive Notes different from Suggested Memories?
Adaptive Notes applies the pattern directly, so there's no suggestion to review.
You can still write your own Memories for anything you want to state outright, and clinic admins can set Clinic Memories for the whole team. See Memories.
â
Is my note still reviewed before it's saved?
Yes. Adaptive Notes only changes how the first draft is written. Nothing reaches your medical record until you've reviewed and approved the note yourself.
â
What if it keeps doing something I don't want?
Keep editing the note the way you want it. Recent edits count most, so the pattern fades.
For something that must always or never happen, add a personal Memory.
Delete any note you edited by mistake so it's no longer used as an example.
As a last resort, turn off "Adaptive notes" in Settings â General.
If you are still experiencing issues with Adaptive notes , contact support with the interaction ID.
â
Is the AI model trained on my edits?
No. The model isn't trained on your edits. Tandem uses your past corrections as examples when it writes your next note for the same template. Tandem's models never train on patient data.
â
Can a note that sounds like me still be wrong?
Yes. Adaptive Notes changes how the note is written, not whether it's correct. It's designed to base every clinical statement on the current consultation only, and you can check each statement against the transcript. Always review the note before you approve it.
â
How do I get access to Adaptive Notes?
New customers have Adaptive notes from the start. If youâve been using Tandem since before September 2026, it becomes available once your clinic's data processing agreement covers it. Contact your Tandem representative if you're not sure whether yours does.
â
Can I turn it off?
Yes. Go to Settings â General and switch off "Adaptive notes". It's on by default and only affects your own notes.
đĄKeep in mind: Adaptive Notes and Memories work together. Adaptive Notes follows the patterns you correct repeatedly. Use Memories for anything that should always or never happen in your notes.
