Note: Creating templates is an evolving feature, and its behaviour may change as we continue to improve it.
You easily manage your clinic's templates and build your own by navigating to Settings > Templates.

Creating templates
You have a few different ways in which you can create a template:
Option 1: Start from a blank template
Option 1: Start from a blank template
If you need full control over the structure of the template, you can start from an empty template and build it step by step.
To create a new template, go to Templates and click "+ Create new template."
This option is recommended if you want to mirror an existing template that you already use in your medical record system.
Option 2: Duplicate an existing template
Option 2: Duplicate an existing template
If you have a template that is almost right but you want to, for example, add or remove something, you can duplicate it to create a copy. This allows you to modify the existing structure and adjust specific sections.
To duplicate a template, open the selected template, click the three-dot menu, and select Duplicate template. A copy of the template will be created, which you can edit without affecting the original.
Option 3: Create a fixed-structure template
Option 3: Create a fixed-structure template
ⓘ Fixed-structure templates are currently available for: EMIS
Fixed-structure templates are used when your medical record system requires a specific structure for notes to be transferred correctly.
This means that you can't modify the overall structure, but Tandem stills allow you to customise how content is written within it.
Read more about how to modify fixed-structure templates here.
Building templates
1. Select primary profession
When you create a template, you’ll be asked to select a primary profession. This helps the AI understand how notes should be written for the template, for example whether it’s mainly used by cardiologists or psychotherapists.
Choose the profession that best matches the intended users of the template. This selection does not affect who can access or use the template at your clinic.
2. Add a name to your template
Give your template a unique name - no two templates can share the same name.
3. Add entries
Start building the template by Adding an entry. For each entry you also need to provide a name. Tandem uses the entry name to understand what information should appear under the entry when drafting a note using the template.
4. Modify entries
After you have added an entry, you have a few different ways in which you can modify the entry:
|
1. Click and drag to change the order of your entries or group entries together under a heading. |
2. Rename the entry.
The entry name is what Tandem uses to understand what information should be written under that entry when a note is generated using the template. |
3. Switch between data types for the entry. |
4. Delete entry |
Data types
Text (regular entry)
Tandem automatically fills this entry with relevant information from the consultation, written as free text.
Heading
Headings allow you to group other entries together under the heading.

Captures content with a number and a unit.

Captures content by selecting one choice from a list.
Multiple options
Used for selecting several items among available alternatives.
Custom structure (advanced)

Example:
ⓘ Templates are saved automatically, so you can always start building a template and return to it later. The template will not be visible to other users at the clinic until you activate it.
Activating templates
Once you have created and finished building your template, or made changes to an existing one, you need to activate it for it to be usable at the clinic.
To activate the template, press the "Activate template" button on the top right. Here you can also select who at the clinic should have access to use the template for patient encounters.
Access options

|
Only me: The template will be available for use only by you. Clinic administrators will still be able to view and edit it. |
Template status
Templates can have different statuses depending on whether they are activated or if you have made changes to them:
|
|
| The template is only visible to clinic administrators. It cannot be used for patient encounters. |
| The template is available for use during patient encounters by the audience that has access to it. |
| The template has been edited, but the edits have not been applied. Users have access to the most recently activated version. |
ⓘ To apply edits you’ve made to a template, press the Apply edits button. The status will change to Activated, and all users with access will have the new version.
🧠 Personalize templates with Memories
Memories help you control how Tandem writes your notes, for example tone, style, and phrasing - without needing to change the template itself. They are especially useful when you want to make small adjustments to how Tandem writes notes or what is included when using a template.
Read more about Memories on the help page Memories.
