Lead capture form / table not appearing

If you’ve enabled Lead Capture but the form isn’t showing up on your Replay page, here are a few quick checks and fixes.

1. Check your plan

Lead Capture is only available on the Marketer plan and above. If you’re on the Free plan, upgrade to unlock this feature.

2. Verify the toggle

Make sure the Lead Capture toggle at the top of your Replay page is switched on.

  • Saving settings alone does not enable the feature.

  • The toggle must be active for the form to display.

3. Review form settings

Open Settings and confirm that:

  • A form title and at least one field are selected.

  • The form was saved successfully.

4. Content lock settings

Check if you locked the entire page or only from a specific chapter. If viewers are watching outside of the locked section, the form will not appear.

5. Branding conflicts

If you customized the Submit button color, make sure it isn’t blending into your page background, which can make the form appear “invisible.”

6. Still not working?

  • Try refreshing the page or testing in an incognito window.

  • Clear cache/cookies if the form is stuck in “saved but disabled” mode.

  • If the issue persists, contact Peech Support with the video page link.


Frequently Asked Questions

Why don’t I see the form when I visit my Replay page?
Lead Capture forms are shown only to new viewers. As the page owner, you’ve already been recognized by the system, so the form won’t appear for you. If you’d like to test it, you can:

  • Open the page on a different browser or device, or

  • Preview the form directly from the settings window.


Will the form appear every time for the same viewer?
No. Once a viewer has submitted their details, Replay remembers them and won’t show the form again on that device. This prevents frustration and duplicate submissions.


Can I preview the form while it’s disabled?
Yes. You can always open the Lead Capture settings to preview how the form looks, even if the toggle is off. The form will only appear to viewers once you enable the feature.


Does the form work on mobile devices?
Yes. Lead Capture forms are fully responsive and will adapt to mobile and tablet screens. On smaller devices, the form may appear as a scrollable overlay to ensure usability.


What happens if I change the fields in my form after people already filled it in?
New viewers will see the updated form, while existing leads stay in your Leads table. Changes won’t affect past submissions.


Can I reset the form so the same user sees it again?
Replay automatically remembers returning users on the same device. If you want to test the form again, open it in an incognito window or on a different browser/device.
