Question types within forms

When creating your form templates, you can choose to use different question types depending on what you're looking to discuss. This enables you to customise your templates to reflect your organisation's review process.

Text Box

This provides a text box for employees to enter their comments.


Ratings

This provides a rating scale made up of numbers or text for employees to select one option from.


You can customise this rating scale either by adding JSON to the question type if the scale is unique to one question. Alternatively, if you'd like the same scale across your account go to Configuration > Forms. You can change the numbers to text, reorder the boxes and choose your desired colours.

Text box and ratings / Text box and appraiser-only ratings

This provides the option for employees to leave a rating and comment in answer to one question. If you select the latter option, the appraisee will only see a text box while the appraiser will also be able to provide a rating that will be seen by the appraisee when shared.

Plain text/HTML

This enables you to add a header or instruction that doesn't require any responses. You can add links to internal documents here too.

Date picker

This allows employees to select a date from the calendar.


Slider

You can customise this percentage slider and it can be used as an alternative to a button rating scale.

Tick box list

This allows employees to select multiple options from a predetermined list.


Number box

This allows employees to only input numerical data.


Two text boxes

This allows the responses to a text question to be split into two columns.

Two number boxes

This allows the responses to a number question to be split into two columns.

Feedback stream

Note: You must have feedback enabled.

If you're using instant feedback or 'get feedback', you can embed the employee's recent feedback stream in forms using the feedback stream question:


9-box grid

If you have a Talent Management section or review, you may wish to include a 9-box grid by selecting the Grid Only question type. This will automatically create the grid below but if you'd like to customise it, please contact support@appraisd.com.


Job description / Job description Alternative

This pulls in the job description that is assigned to the employee on the People tab. The review will retain the job description the person was assigned at the time the review was created.

Job description Alternative will pull in the "Secondary job description" for the user (if enabled).

Note: if you assign one after the review is in progress, it won't automatically update.

File upload slot

This allows your employees to upload a file. Files can be attached to other text box questions but this option has a specific file upload button.


​Calculated score

This allows you to have an automatically calculated average score based on ratings in the current section of the form. It will generate a score based on the current user's ratings so if the question input is set to employees and managers, and both these people are required to answer rating questions in the section, you will see two scores in this question:

  1. Average score based on the employee's self-assessment ratings

  2. Average score based on the manager's ratings

If you'd like only manager's to generate an overall score, you can set the question input to "managers and feedback-givers" (score will be visible to the employee) or "managers can input, hidden from employee" (score will be hidden from the employee).

Nominate a colleague + comment

This allows your employees to search for another user on your account and add a comment. The email address of the tagged user will show in the reports for this question.


​Feedback invitation list + invite button

Note: You must have request feedback enabled.

The Request feedback button will be available on the left panel of the review and on the review tile. However, this question type allows you to embed the invitation list and invite button somewhere within the review itself if you'd like to make it more prominent.
