The Atlas Pharmacy system reviews the details of all prescriptions received from Titan for each item against the patient’s current “Repeat med” file (their eMAR chart). If there are any new drugs or differences between the prescription and the current therapy, then a check is created in the Clinical check section. These checks are categorized by allocation of the prescription item;
A:Urgent Checks - Items already handed out
B:Interim Order Checks
C:Other Checks (Items without active order)
D:Monthly Order Checks
All checks need to be completed in a timely manor, delays in completion will effect the data displayed to the home on the Atlas device also the ability to book in and scan the medication label.
The checks can be accessed by clicking clinical checks on the home page or the clinical check tab.
To view action a check following this process;
1.Double click on the pending check to load the checking screen. The screen shows a image the prescription on the right hand side, you can also access the full patient record if you wish to by clicking the “…” next to the patient name.
2. Any proposed change to the patients record will be displayed in the from and to section of the screen. You are also able to see the items current administration schedule in the from section (if applicable).
3. You are then prompted to check particular things such as start date and dosage requirements. These can be viewed under Recommendations. The recommendations vary depending on the type of change:
For dosage changes you are firstly requested to consider if you want to change the care home administration schedule on the care home’s e-MAR system. – The check will prompt the following request, "do you wish to change the repeat med record or stick with the existing?" If there is actually no change to the administration schedule but the dosage wording has changed then you can simply select stick with current dosage and times, no further action will be required.
For New drugs or dosage changes that you want to change the schedule you are requested to answer the following:
When will the dosage change take place? (for new drugs immediately, for dosage changes the next day.)
(Setting a dosage change to take place immediately may result in the item re-prompting for administration on the care home's e-MAR as a result of the dosage being changed)
Enter a dosage selected from the system so that the drug prompts on the e-MAR.
If you wish to approve the check, search the appropriate dosage and allocate the relevant times where appropriate. Once completed select approve.
If you wish to decline the check select decline. For any dosage changes the current therapy (displayed in the from section will remain active, for any new drugs declining the check will result in the item being marked as archived.
If you have a query regarding the therapy mark the check as verification needed. This will separate the check and start a new section called Verification needed. This allows you continue with other checks and review it at another time. You are able to leave a note in the dedicated box if you wish to.
As a default setting for all pharmacy checks are created before the dispensing process on Titan can be completed this however can be amended so that all checks are created once the item has been handed out on Titan.
If you wish to change this setting please follow this process;
1. Log in to Atlas Pharmacy.
2. From the main menu, click computer settings.
3. Click the pharmacy tab, then remove tick in box next to Create therapy checks before hand out within the Titan integration settings.
Tips
We recommend that checks for Drug interactions, allergy queries, increasing or decreasing dosages and the stopping and starting of medication on the repeat med record are reviewed by a pharmacist.
Separate checks awaiting review by a pharmacist by using the verification needed status and the note functionality.
