An overview of Atlas Scorecard

Atlas Scorecard - Operational management tool to review your organisations stats such as, booking in of medication, bar code scanning success, CD witness success and lots more.

Scorecard will default to show Data from the last 7 days for All units. You can amend the time period & unit you would like to view from the top of the page.

Top Level Reporting

Score in 12 key areas
Default targets are allocated, but can be amended to reflect each home’s needs.


Example:
A home with a high percentage of Respite Beds will require a lower Barcode Scanning Percentage.

Criteria Analysis

Barcode Scanning Success

Displays:

- Unit
- Each member of staff
- Time period
- Medication round


With the time period you can click on any of the columns in the other sections and see the data behind it.

The percentage is calculated on medication recorded as given. Medications marked as not given do not have to be scanned if not given. This will not affect the barcode scanning success.

Round Level Data

Barcode Scanning – Afternoon Rounds
Having clicked on the afternoon round from the round time option, you will be shown ALL of the medication rounds that took place associated with this time.


This shows:
- Drugs accounted for (potted)
- Drugs Given
- Drugs recorded as Not Given
- Number of Given drugs barcode validated
- Percentage of Given drugs barcode validated
- Is the round a pass or a fail?


To see the round in detail, click + on the left

The round level data displays:

- Which items are scanned
- Which item are not given & why
- The initials of the resident for each item
- The time the round started
- The time the round finished
- Time between residents


By clicking on the meds details to the right of an item you will see the admin history of that item for the item period in use.

Medication Details

Not Given - Too Soon
In this example I clicked on the top ‘Not Given – Too Soon’


The prior dose of analgesia was administered at 09:51


The Not Given – Too Soon is recorded at 12:58 (3 hrs and 7 minutes later)


The next administration is at 17:01. Meaning for the sake of 53 minutes, the resident has gone 7 hours and 40 minutes since the previous administration.


Evidence the member of staff at lunchtime was focused on the process of finishing the meds round, rather than being Person Centred and returning to offer the medication once the 4 hours are up.

This is the percentage of items that prompted (turned RED) that have been accounted for. Any not accounted for are missing entries. In this case the home has accounted for 4275 items in the last week with 19 missing entries.

Rather than having to look through every item in the homes MAR charts ATLAS Scorecard displays only the medication rounds which contain a missing entry.

Administration Success

Administration Success

For this criteria we are reporting on the % of successful administration ATLAS eMAR did not need to
intervene to avert a potential meds error.

NB the higher barcode scanning percentage the more effective this criterion is.

Interventions include:

- Not 4 hours gap on analgesia
- Drug has been stopped
- Medication has already been given
- Medication selected too early
- Wrong resident’s medication
- Invalid barcode scanned

Safe Time with Patient Success

Safe Time with Patient Success

This measures the length of time from selecting the last item (turning to Yellow), to the time the member of staff signed the eMAR, to confirm they have witnessed the resident having taken their meds.


To pass, there must be at least 20 seconds, where at least 1 item is taken.

Reasons for failing:
- Signing the eMAR then giving the meds
- Using Runners
- Pre-Potting meds
- Meds have been given from memory, and records updated later

The two graphs demonstrate clearly that the home has an issue with Night Staff, as well as Early Evening
medication.


In addition, you can see how each member of staff is performing. It is often the case that a member
of staff failing on the Safe Time with Patient Success, will also fail the same round on Barcode Scanning
Success, indicating that they are not following the procedures for the use of ATLAS eMAR and your company’s Medication Policy.
