How it works
1. Codes appear during the visit
As Tandem captures the consultation, relevant diagnosis and procedure codes appear automatically, drawn from what's said in the room.
2. Ranked for your framework
Suggestions are ordered by relevance and matched to the level of specificity your framework requires.
3. Review, confirm, and send
Check each suggested code, adjust if needed, and send them to your medical record system with the note. Nothing reaches the record until you confirm.
What it does
Suggests diagnosis and procedure codes from your active framework. Works with ICD-10, ICPC-2, KVÅ, SNOMED CT, and more, depending on your market.
Search for any code. Use the search bar in the coding panel to look up codes by name or number and add them to the visit, whether or not they were suggested.
Context-aware. Accounts for visit details like patient age, encounter duration, and clinical context, so suggestions fit the case rather than defaulting to the most common code.
Shows related codes. Associated codes appear alongside the primary suggestion, so secondary and supporting codes are easy to see.
Navigate to more specific codes. Move past unspecified defaults to related, more detailed codes when the visit warrants it.
Catches invalid combinations. If two codes don't fit together, Coding Assistant flags it so you can resolve it while the visit is still open.
Flags missing supporting codes. When a diagnosis needs a supporting code (such as an external cause code for an injury), Coding Assistant flags it before the note is signed off.
Clinic settings for coding
Clinic admins can tune how Coding Assistant behaves for everyone at the clinic. Go to Settings → My clinic → Clinic settings, then scroll to the Coding section.
Manage pinned codes. Pin frequently used codes so they're easy to find for everyone at your clinic. Pinned codes appear in the Pinned tab during any visit, one click away.
Require external cause codes for injury diagnoses. When on, Coding Assistant prompts for an external cause code whenever an injury diagnosis is added.
Warn when transferring without codes. When on, Tandem warns you before transferring a note to the medical record system if no codes are attached.
These settings apply to everyone at the clinic, not just the user adjusting them.
Good to know
Suggestions come from documented context only. Coding Assistant suggests codes for things in the consultation. It doesn't invent codes for content that wasn't documented.
You always confirm. Nothing reaches the medical record without your review. This is a regulatory requirement and a clinical safety choice.
Class IIa under EU MDR. Coding Assistant is CE marked as a Class IIa medical device under the European Medical Device Regulation. An independent body reviewed the clinical evidence and the AI risk controls.
Framework availability varies by market. Coding Assistant is available where the framework is certified for your market. The list of certified frameworks updates as new certifications complete. If you're unsure about your market's status, ask your account team.
Common questions
Why didn't Coding Assistant suggest a code I expected?
Coding Assistant suggests codes for things documented in the consultation. If something wasn't said or recorded, no code will appear. You can always add a code yourself: search by code name or number in the coding panel and add it to the visit.
What happens if I disagree with a suggestion?
Don't accept it. Only confirmed codes are saved. If you see a pattern of incorrect suggestions, tell your account team. Feedback shapes how the system improves.
Does it work with my medical record system?
Coding Assistant works with any medical record system Tandem integrates with. Confirmed codes transfer with the note when you save the visit. If you're not sure whether your medical record system is connected, ask your clinic admin or see our Integrations page.
How is patient data handled?
All processing happens in European data centres. Audio is processed in real time and deleted immediately after transcription. Tandem doesn't train models on patient data.
Why aren't codes applied automatically?
Codes affect the medical record and reimbursement, so clinician judgement is required. Under the EU AI Act's human oversight requirements, this is also a regulatory expectation. In practice, confirming a ranked suggestion takes one click.
Can my clinic turn off Coding Assistant?
Yes. Contact your account team to disable it. There's no self-serve option; Tandem disables the module from our side.
Is Coding Assistant available on the Tandem mobile app?
Yes. Coding Assistant works on the Tandem mobile app the same way as on web.
