How to off-board (and reactivate) patients from Huma

This help guide walks you through the patient off-boarding and reactivation process in Huma

Patients who no longer require remote monitoring should be off-boarded from Huma.

If a patient's health status changes and they subsequently need to be monitored again, you can reactivate a patient and continue to monitor them on Huma.


How to off-board patients from Huma

  • You can off-board patients from the patient list

  • Scroll over and click the additional options menu i.e. the three dot menu (...) besides the patient name

  • You will see an option named 'stop monitoring patient'. Click on this to start the off-boarding process

  • An off-boarding pop-up screen will appear asking you to confirm patient off-boarding and choose a reason for off-boarding

  • Click 'Stop monitoring patient'

  • A pop-up notification will appear on screen to notify you that the patient has now been off-boarded


What happens to off-boarded patients in the Huma Portal?

Patients that are off-boarded continue to display on the patient list and will appear further down the patient list i.e. after actively monitored patients and appear visually different i.e. faded / greyed out to help distinguish these patients from patients you are actively monitoring - see below

You can also filter and view 'all off-boarded patients'.

  • Click on the 'filters' button at the top of the Huma portal

  • Under 'patient type' click on 'off-boarded patients'

  • Click on 'search'

  • You will now see a filtered view of the patient list, displaying all off-boarded patients


What happens to patients who are off-boarded from Huma?

Off-boarding a patient from Huma triggers a cascade of off-boarding events:

  1. The patient will receive a Huma generated Email communicating to them that their account has now been closed

  2. The patient will also receive a push notification on their smartphone to notify them that their account has now been closed

  3. If a patient opens the Huma app, a pop up screen will appear communicating to the patient that their account has now been closed. Patients will be asked if they would like to download their data before logging out. Once off-boarded and logged out, patients will not be able to submit data using the Huma app


How to reactivate patients in Huma

If a patient's health status changes and they subsequently need to be monitored again, you can always reactivate a patient and continue to monitor them on Huma.

  • To reactivate a patient, filter 'all offboarded patients'

  • Click on additional options menu i.e. the three dot menu (...) besides the patients name

  • Click on the 'Re-activate patient' button

  • A pop up menu will appear asking you to confirm patient re-activation. Click 'Re-activate patient'

  • A pop-up notification will appear on screen to notify you that the patient has now been re-activated


What happens to patients who are re-activated on Huma?

The patient will receive a Huma generated Email communicating to them that their account has now been reactivated

  • The patient needs to download the Huma app (if they have deleted it from their smartphone)

  • Within the reactivation email, the patient must tap on the 'open Huma' button. This will open the Huma app and prompt the patient to sign in

  • Once the patient has signed in, they can continue submitting health data!
