Setting Up Form Restrictions

Set up control over your Forms

On Chime, you can apply form restrictions using cohorts to control:

  • Who can complete the form

  • Who can view the completed submissions

This is especially useful for managing sensitive or confidential data, such as medical or compliance records - giving you more control over access and visibility.


Restricting Who Can Complete the Form

  1. Go to the Forms tile

  2. Open an existing checklist or start a new one

  3. On the left-hand side, under Tags, you'll see a section called Cohorts

  4. Click the Select Cohort icon

  5. Choose your cohort(s) from the pick list (you can select more than one)

  6. If left as Everyone, all users will be able to complete the checklist

  7. Once selected, only the specified cohort(s) will have access to complete the form

Restricting Who Can View the Completed Form

  1. In the same form, open up the Additional Options section on the left

  2. Tick the box labelled Restricted

  3. A new field will appear: Viewable by Cohorts

  4. By default, this is set to Everyone

  5. Click the icon to choose a specific cohort that can view submissions (e.g. “Super User Restricted”)

  6. Only users in this group will be able to access submitted data

Final Step

When you're done setting restrictions, press Save & Close.

Your checklist will now respect both completion and viewing permissions based on the selected cohorts.


Use Case

Setting up restrictions is typically done by Super Users or Admins when creating or managing forms. It ensures that the right people complete the form, and only authorised users can view the results.

  • Create a dedicated restricted-view cohort for sensitive forms
