At times, you may require paper-based forms for submitting applications, requests or queries to our Customer Experience Team. We offer a range of forms for various purposes, as outlined below:


Learner Registration & Certificate Application Forms

It is the responsibility of our Training Providers to handle all learner registrations and certifications through our Online Services system. However, if there is a requirement for a paper-based form, the appropriate forms available for download below.


Learner Registration Amendment Form

To request amendments to a unit on a learner's registration, please download and complete the following form. Please note that Online Services can be used to transfer learner pathways. For pathway transfers, please refer to the relevant article.


Bulk Registration

If you need to upload multiple learner registrations, please download and fill out the below Excel spreadsheet. For guidance on how to upload the spreadsheet, please refer to this article.


Post Return Form

Just so you know, we no longer accept postal mail sent to our head office. All communications must be sent via email. If you need to return any mail to us, please download the form below, complete the information you need and submit it along with your enquiry to our Customer Experience Team.


Learner Centre Transfer

Please download and complete the form below to transfer a learner from another centre. This form can only be submitted by the Centre Coordinator.


It is important to note that these forms must be submitted by an authorised user with the appropriate permissions or by the Centre Coordinator, as mentioned above. The form may be rejected if you do not have the necessary authorisation.
